Home and Community Care and Longterm Care Program
The Home and Community Care Program provides home and community based health services to assist individuals of all ages with chronic and acute illness, disabilities, and the elderly to receive care in their homes in Oneida.  The program delivers coordinated health and community support services and works in partnership with clients and families in care planning to maintain optimum health, well-being and independence in their home and the community. Home and Community Care services are provided by Registered Nurses, certified Personal Support and trained Home Support Workers. Registered Nurses support and supervise the Personal and Home Support workers.  Program services include: client assessment service; case management; referrals and links with other community support services and external healthcare services; Loan Cupboard for short term needs (medical equipment).  Personal Care support activities include:  bathing, grooming; dressing; simple dressings, blood pressure/blood sugar and medication monitoring.  Home support activities include light housekeeping, laundry, meal preparation. 
The Long Term Care Program is coordinated by the Home Care Program to provide service to individuals (adults and seniors) with a chronic illness and disabilities. Program services include:  adult day programs (bingo/crafts/meals), fitness/exercise program, weekly congregate dining, in home exercise day trips (includes shopping/special events/movies), annual senior’s picnic and Christmas event.
